General Care Management

Care managers assist patients and their families with the management of medical conditions, psycho social concerns, and coordination of care with the goal of improving the patient’s health and their overall quality of life.

Comprised of health professionals (RN/LPN/LMSW) located at our primary care offices, the role of Oaklawn’s care managers also includes working with patients and their families to develop health goals and a patient-centered plan of care, while also providing education and support that includes assisting with referrals to community resources.

In addition to Oaklawn’s care managers, dialing 2-1-1 is a simple way to connect people with human services for support with things like utility assistance, food, clothing, housing, and more.

Chronic Care Management for Medicare Patients

Oaklawn has partnered with CareHarmony to provide support to Medicare patients living with more than one chronic condition (arthritis, diabetes, depression, high blood pressure, etc.). With the support of your primary care physician, your dedicated CareHarmony care coordinator will monitor your day-today wellness and help you stay as healthy as possible between office visits. Available 24/7, your designated care coordinator will help coordinate appointments with specialists, assist with medication management, and more.
