Oaklawn and CareHarmony Mark One Year of Partnership Supporting Patients with Chronic Conditions

Marshall, Mich. — One year ago, Oaklawn partnered with CareHarmony to enhance care for Medicare patients living with multiple chronic conditions. Today, that partnership continues to thrive—delivering personalized, proactive support that bridges the gap between office visits and helps patients stay healthier, longer.

To date, more than 1,000 Oaklawn patients are currently enrolled in the program, reflecting a strong demand for compassionate, continuous care outside of traditional clinical settings.

Through this collaboration, eligible Oaklawn patients are connected with a dedicated CareHarmony care coordinator who works directly with their Oaklawn primary care provider. This team-based approach focuses on chronic care management (CCM), providing consistent, compassionate help tailored to the patient’s individual needs.

Care coordinators are available 24/7/365 through a dedicated Care Coordinator Line, helping patients with questions, coordination, and additional assistance—any time, day or night. CareHarmony’s team also reaches out directly to eligible patients by phone to explain the service, answer questions, and help them enroll.

“CareHarmony has become an extension of our care teams,” said Chapman, “They’re helping us deliver ongoing support that keeps patients on track with their health goals and, most importantly, out of the emergency room.”

Patients with conditions such as arthritis, diabetes, high blood pressure, COPD, and other chronic illnesses benefit from services that include:

  • Assisting with medication refills and adherence
  • Helping with transportation and access to community resources
  • Offering education and encouragement between visits

This preventative, wraparound support model helps reduce hospital admissions, avoid unnecessary emergency visits, and improve overall patient outcomes—especially for those managing multiple health challenges.

The service is covered by Medicare and may be subject to a patient’s deductible or coinsurance, typically 20%. Many with secondary insurance plans have little or no out-of-pocket costs. Participation is voluntary, and patients may opt out at any time.
