Health Information Management / Medical Records

Medical records can be obtained by filling out a Release of Information form and emailing it to medicalrecords@oaklawnhospital.com or faxing to Health Information Management.  You can also call the Health Information Management office for assistance with this process.

Health Information Management is located on the first floor of the main hospital building.

Cost:  There is no charge for patients to obtain a copy of their medical records.
