Understanding Lymphedema: Causes, Symptoms, and Treatment Options

Lymphedema is a condition that affects millions of people and can significantly impact daily life if left untreated. Understanding the signs, risk factors, and available treatments can help you take action early and manage symptoms effectively.

What is Lymphedema?

Lymphedema occurs when protein-rich fluid accumulates in an area of the body due to abnormalities in the lymphatic system. The lymphatic system helps transport fluid and filter waste throughout the body. When lymph vessels or lymph nodes are missing, damaged, impaired, or removed, fluid cannot move properly, causing swelling and fluid buildup.

Where Can Lymphedema Occur?

Lymphedema can develop in several areas of the body, including:

  • Arms
  • Legs
  • Face
  • Neck
  • Trunk
  • Genital area

Who Is at Risk?

Lymphedema and other forms of chronic edema can develop for a variety of reasons, including:

  • Cancer treatment or surgery
  • Venous (vein) disorders
  • Obesity
  • Heart conditions
  • Kidney disease
  • Genetic predisposition

For some individuals, lymphedema related to cancer treatment may appear within months of treatment, years later, or sometimes not at all.

You may be at increased risk if you have undergone:

  • Simple or modified radical mastectomy with axillary (armpit) lymph node removal
  • Lumpectomy with axillary lymph node removal
  • Cancer surgery and/or radiation therapy involving lymph node regions such as the neck, armpit, groin, pelvis, or abdomen

What Are the Symptoms?

Common signs and symptoms of lymphedema include:

  • Swelling that may leave an indentation when pressed (known as “pitting”)
  • Swelling in the arms, hands, fingers, shoulders, chest, or legs
  • A feeling of fullness or heaviness in an arm or leg
  • Tightness of the skin
  • Reduced flexibility in the hand, wrist, or ankle
  • Difficulty fitting into clothing in one specific area
  • Jewelry, watches, or rings feeling tighter than usual

If you notice any of these symptoms, it is important to speak with your healthcare provider.

Does Lymphedema Go Away?

While lymphedema is not curable, it is highly manageable with proper treatment. Early intervention can help reduce symptoms, improve comfort, and prevent progression.

How Is Lymphedema Treated?

The gold standard treatment for lymphedema is Complete Decongestive Therapy (CDT). This treatment is individually designed by a certified lymphedema specialist and typically includes:

  • Manual Lymphatic Drainage: A specialized massage technique that helps move excess fluid out of the affected area.
  • Compression Bandaging: A specific wrapping method that applies pressure to encourage fluid movement and reduce swelling.
  • Compression Garments: Once swelling has been reduced, compression garments help maintain results and manage swelling long-term.

Your therapist will develop a treatment plan tailored to your specific needs and goals.

Can’t I Just Take a Water Pill?

While diuretics (water pills) may remove some excess water from the body, they do not remove the protein that accumulates with lymphedema. This protein contributes to inflammation and tissue changes and continues to attract additional fluid. As a result, water pills alone are not an effective treatment for lymphedema.

What Should I Do If I Think I Have Lymphedema?

If you suspect you may have lymphedema, consult your healthcare provider. Your doctor can perform testing to determine the cause of your swelling and recommend appropriate treatment options.

If Complete Decongestive Therapy is recommended, your physician may refer you to a certified lymphedema therapist.
