What is a Patient-Centered Medical Home? 

Patient-centered medical home refers to a national healthcare initiative that’s dedicated to providing more accessible care to patients. It’s an extension of the in-office partnership you have with your physician and office staff. 

Our goal is to make it easy and comfortable to provide you with care in a way that works best for you. We offer multiple ways for you to discuss your health issues with us, such as extended office hours, open scheduling, and through your patient portal. 

Our medical home offers timely access to care even when we are not open. Simply call your physician’s office and you will be provided with contact information for the on-call provider. Through our office, a physician is always available to answer your questions 24 hours a day, 7 days a week.

When is it an emergency? 

Call your primary care provider’s office for non-emergency illnesses like: 

  • Cold 
  • Influenza
  • Non-debilitating headache
  • Back pain
  • Low-grade fever
  • Sore throat
  • Diarrhea

Call 911 or go directly to the nearest ER: 

  • Severe allergic reactions 
  • Severe burns
  • Broken bones
  • Head injury
  • Loss of consciousness
  • Poisoning and overdoses
  • Severe bleeding
  • Severe pain or headache
  • Chest pain or pressure
