The OAKLEAF Award Nomination Form

The OAKLEAF Award is presented quarterly to one clinical and one non-clinical Oaklawn employee who exemplifies the kind of skill, professionalism and care our patients, families, and fellow employees deserve, such as:

Making a special connection with a patient or family
Demonstrating excellence as a team player
Serving as a role model to other employees patient’s, families, visitors, or fellow employees may nominate a deserving member of Oaklawn’s staff

Each OAKLEAF Award honoree will be recognized at a public ceremony in his/her department and receive a framed certificate of recognition, OAKLEAF pin, and engraved glass award.
